Support Coordination

Working through how to make the best use of your NDIS funding can be a challenge. Finding the right service providers to meet your needs, coordinating different services and keeping track of your expenses can be overwhelming.

We reconise these challenges at Sunflover Care Victoria. As your support coordinators, we can:

• help you to understand your NDIS plan
• link you to the suitable service providers
• support you to achieve your goals
• monitor your budget and services
• help you make connections to your local communities.

We do Support Connection, Coordination of Supports and Specialist Support coordination.

Psychosocial Recovery

Sunflower Care Victoria provides NDIS participants coaching in Psychosocial Recovery.

As Psychosocial Recovery Coaches will support participants to take more control of their lives and to better manage the complex challenges of their day-to-day living.

We will work collaboratively with participants, their families, carers and other service providers to design, plan and implement a recovery plan, and assist with the coordination of NDIS and other supports.

Our aim as psychosocial recovery coaches is to help build an individual’s capacity across all areas of the person's life.

Therapeutic Therapy

Sunflower Care Victoria provides Therapeutic Therapy services such as counselling, assessment, recommendation and training for the participants as well as their carers or parents.

The NDIS values counselling for people who live with disability. Counselling is available in the NDIS under Improved Daily Living Skills.

The NDIS also recognises the importance of support in the areas of assessment, recommendation and training, including training in assistive technology, for people who live with disability.

On top of these therapies for the participants, training for carers or parents in matters related to caring for a person with disability is also covered by the NDIS.

Community Nursing Care

Some NDIS participants are eligible for community nursing care.

At Sunflower Care Victoria, we provide in-home nursing support such as:

• Medication management
• Diabetes management
• Stoma care
• Bowel Care

Our aim is to help you manage your day-to-day healthcare needs in the comfort of your own home.

Aged Care

Although this service is not funded by the NDIS, we recognise the need for assistance in aged care in the wider community.

At Sunflower Care Victoria, we provice Therapeutic Therapy services such as individual counselling service for aged care clients and their carers to work through lifespan changes, as well as grief and loss events.

We also provide in-home nursing care support to elderly clients to help with medication management, diabetes management, stoma care and bowel care.

Our ability to communicate with our elderly clients in Mandarin, Cantonese or Hakka is valued by many.
